The rotating cuff muscles and tendons are built to rotate our shoulder (and thus their name) and to lift our arms above our head. The bursa or bursal sac is intended to protect the muscles against scraping on the bone edge. The bursa therefore sits between the muscle and the bone, functioning like a coil. Without it the rotating cuffs will scrape on the edge of the bone again and again and become irritated. The shoulder capsule gives the shoulder joint more stability. Any of these structures may be an inflammatory or injured pain generator. Sometimes in combination with each other, they are inflamed.
